How the anomaly system works
In Animal Hospital you work the night shift, checking animal patients in at a reception window. Most are ordinary — but some are anomalies in disguise. Let the wrong one inside and you lose sanity, and a serious mistake can end your run. Every anomaly reveals itself through one of three checks, which is why the routine below matters so much.
| Step | What you're checking | Catches |
|---|---|---|
| 1. Appearance | Look at the patient at the window for 4–5 seconds. | Appearance anomalies |
| 2. Photo | Take a picture and let it fully develop before touching it. | Photo anomalies |
| 3. CCTV | Sweep the security feeds before opening the shutters. | CCTV anomalies |
| 4. Inside | Deal with threats that roam the hospital. | Monsters |
What is Animal Hospital?
Animal Hospital (also known as Animal Hospital (Anomaly)) is a co-op horror game on Roblox. It looks like a cosy veterinary simulator, but it is really a survival-horror experience: you work the night shift with up to three other players, checking animal patients in at a reception window, diagnosing them and giving the correct treatment — while identifying and denying anomalies disguised as ordinary animals.
The tension comes from never quite knowing whether the next patient is genuine. Admit the wrong one and you lose sanity, trigger a hostile monster, or end your run outright. This wiki documents every anomaly, item, treatment and mechanic so you can clear each shift with your sanity — and your patients — intact. See the latest updates for what's new.
Frequently asked questions
What is an anomaly in Animal Hospital?
In the Roblox game Animal Hospital, an anomaly is a patient or event that is not normal — a creature disguised as an animal, a corrupted photo, or a monster inside the hospital. Your job on the night shift is to spot anomalies and deny them entry before they cost you sanity or end your run.
How many anomalies are in Animal Hospital?
This guide documents 30 anomalies and special characters across four types: appearance, photo, CCTV and roaming monsters. The developers add new anomalies over time, and we update this list as they appear.
What order should I check each patient in?
Check appearance at the window first, then take a photo and let it fully develop, then sweep the CCTV feed, and only open the shutters once the patient passes all three. Skipping any step is the most common way to let an anomaly slip through.
What is the hardest anomaly to spot?
The Skinwalker is the most dangerous because it looks completely normal at the desk and in photos — only the CCTV reveals it. Among the subtle ones, mismatched ears (in both photo and camera form) are the easiest to miss.
