Because there is no obvious deformity, this anomaly punishes players who only take a static, front-on look and then reach for the shutters. The movement is the entire anomaly, and you will only see it if you actually move.
To expose it, shift your position slightly while you assess the patient. If the eyes track you across the window, that following motion is your confirmation — refuse entry.
